Replicate of Birth Certificate of youngster provided by Ministry of Health, UAE Government-- With English translation appropriately proved by MOFA as well as MOHA, UAE Government.

Copy of the Marriage Certificate of the moms and dads;.
Passport Application for brand-new birthed kid;.
Duplicate of Emirates IDs of both moms and dads;.
